The form entitled “Ebösszeírási Adatlap” can be downloaded from the debrecen.hu website. Alternatively, citizens can submit the form electronically via the “Ügyfélkapu” (digital citizen system).
The form can be picked up and submitted in person at the New City Hall building (Debrecen, Kálvin Square 11.) during office hours.
The form can be submitted by post as well to the following address: Debrecen Megyei Jogú Város Polgármesteri Hivatala, Igazgatási Osztály; 4026 Debrecen, Kálvin tér 11.

Dog censuses must also be completed for microchipped dogs already on the central register and for dogs registered during or after previous censuses.
